"I would like to enroll in the Ophthalmic Career Progression Program (OCPP), administered by the National Academy of Opticianry. I understand and agree that I am entitled to the complete OCPP course based upon successful completion of each Volume exam, Final examination (receiving 70 percent or above on each) and payment of appropriate fees. To receive a certificate of completion upon successful completion of the program." Upon verification of eligibility, Luxottica Retail will make payments to the National Academy of Opticianry on my behalf as follows: Phase I: Payment of $400.00 will be made for Volume One, by Luxottica Expense Payables, to the National Academy of Opticianry. Phase II: Upon successful completion of Volume One Exam, payment of $700.00 will be made to the National Academy of Opticianry, by Luxottica Expense Payables, for the remaining Volume Two, Volume Three, Volume Four and Final Examination. Upon receiving this application, the National Academy of Opticianry will invoice Luxottica for the initial $400.00 payment on behalf of the student. However, no books or letters will be mailed to the student until the National Academy of Opticianry receives the initial $400.00 payment from Luxottica.
